Sports Physicals for DCS Athletes

All students who wish to play sports next year at Deckerville Schools are required to have an annual physical examination and an MHSAA sports physical card on file in the Athletic office.

From June 15 – July 15, participating physicians at Deckerville Health Care Services will be providing Sports Physicals at a cost of $5.00. You must make an appointment ahead of time. Walk-ins will not be accepted.

Physical cards may be picked up in the high school or athletic office, or obtained in the physician’s office. These cards must be filled out and signed by the parent before the appointment.
